Skilled Nursing Facility
The Skilled Nursing Facility (SNF), located on the second floor
of Shamokin Area Community Hospital, is focused on the next step
of recovery, the transition between inpatient hospital care and
going home. Whether recuperation involves physical therapy or a
more intensive treatment, it is our primary goal to see guests return
home as a result of a personalized plan of care that involves care-givers,
the guest, and family members.

In the Transitional Care Unit at Shamokin
Area Community
Hospital, we provide personalized care in a comfortable,
home-like environment. Each individualized plan
of care is developed with the needs of the guest
in mind.
Visitors are welcome 24-hours a day,
without limitations, even pets can visit! Nutritious,
appetizing meals are
provided in the dining room, and a wide variety
of activities are available to participate in
to enhance recovery.
Each person's recovery is different. Guests in
the SNF are able to utilize the many services available at Shamokin
Area Community Hospital if their situation warrants it. In addition,
if a guest requires further care once they are discharged from the
Transitional Care Unit, the social services staff will help accommodate
those needs as well.
Admission Information:
- Physical
or occupational therapy or other skilled
nursing services must be prescribed.
- An admissions
team will review medical history.
- Accepting
most major insurance plans including:
Advantra, Capital Blue Cross,
Gateway, Geisinger Health Plan, Health
America, Highmark Blue Shield, Keystone, Keystone
Senior Blue, Medicare,
Sterling, and United Health Care.
- Average length
of stay is 10-20 days.
